Covid-19 Update

To Our Patients Regarding COVID-19:

UPDATED AS OF June 1, 2020:

All Patients must call/text the office upon arrival and remain in vehicle until directed by a staff member to enter.

All patients must wear face covering (mask) while in office except when directed by clinical staff to remove.

All patients will be asked health screening questions & temperature will be taken before entering office.

All patients must come to appointments alone unless other arrangements have been made in advance.
